The Application:

  • The application form presents the essential elements of information concerning the organization and the particular certification program(s) candidates for accreditation.
  • Part I of the application details the organization's key information.
  • Part II of the application presents key information on the certification program for accreditation. (Note:  if more than one program is being evaluated, a Part II must be completed for each and every program for accreditation.)
  • The submitter of the application should be the organization's President, CEO, or Executive Director.
  • Initial fees required for the accreditation are identified on the reverse of the application form.
  • The Required Exhibits:  The required exhibits are in two (2) phases.  Phase I concerns the organization.  Phase II exhibits represent the particular certification program being evaluated.  Phase II exhibits are required for each program being considered for accreditation.  Each required exhibit should be "tabbed" to conform to the alphabetical or numerical designation of the exhibit for Phase I and Phase II.
  • Phase I Exhibits:
  • Documents that show the structure of the organization, such as:
  • Articles of Incorporation
  • IRS profit, non-profit, not-for-profit determination letter
  • Year end financial statement (optional)
  • A list of all officeholders and/or staff
  • A copy of the bylaws
  • A copy of policies and procedures
  • A copy of any promotional literature
  • Membership types and fees associated with each
  • A detailed list of all certifications and services offered by the organization
  • Phase II Exhibits:  Separate Phase II exhibits are required for each certification program offered as a candidate for accreditation.
  • A copy of the certification program handbook (literature)
  • Specifics on the certification process
  • A listing of the certification standards against which certification eligibility is measured
  • A copy of all certificates issued
  • A copy of the certification "application package"
  • List of study materials available
  • Certification fees and certification duration
